The most common way for people who are turning 65 to apply for Medicare is with a simple online application located on the Social Security website. The application process is relatively straight forward and takes about 5 to 10 minutes to fill out. Here is the link to the application on the Social Security website.

Apply By Phone

To apply by phone, simply contact Social Security at 1-800-772-1213 and let the representative know that you would like to apply for Medicare. Depending on your personal situation, they may need to mail you some forms to complete. These forms are generally pretty simple, but if you have any questions, feel free to give us a call. Apply By Mail

You can apply for Medicare by mail

Mailing a paper application is the best option for those over 65 who have Medicare Part A and wish to apply for Part B. If this applies to you, and you are currently leaving credible group coverage there are two forms you will need to complete and mail to your local Social Security office. Here are the links to them below:

Part B Application: CMS 40-B

Proof of Coverage Form: CMS-L564

Apply In Person

Applying in person at your local Social Security office is the best option for those who are turning 65 soon and need to get their application processed quickly. You can find the closest office near you by typing in your zip code here. Be sure to ask for a printed confirmation showing you have enrolled in Part A and Part B when you are at the Social Security office. This confirmation will allow you to go ahead and complete your Medicare Supplement and/or Part D applications.
